We provide application assistance for Medicaid in the form of presumptive eligibillity.  This provides the individual assistance with applying for temporary and long-term Medicaid insurance.   For more information, please visit the KDHE link for Medicaid Information for application information.

We are grateful to be a part of the Kansas Chronic Disease Risk and Reduction grant, funded through the Kansas Department of Health and Environment.  The Cherokee County Health Department provides assistance in three work plan areas: Tobacco Prevention, Physical Activity and Nutrition.  We strive to provide methods that prevent, promote and protect our community for future generations of health.

The Tobacco Cessation work plan focuses on aligning prevention efforts for smoking and tobacco prevention among all age groups and populations, policy making, and advocacy to our community.

The Physical Activity work plan provides guidance for resources that help promote healthy activities in the form of physical activity within communities.  Such projects have included the development of the West Mineral Park, assistance with resources for the Weir City Park and continued collaboration with other cities located in Cherokee County Kansas.

The Nutrition work plan provides assistance, guidance and advocacy for lactation promotion within our community.

All three work plans focus to create advocacy within our local coalitions, continuing to provide the three principles of public health: prevention, promotion and protection.
